High Plains Senior Care Hospice in Amarillo, TX is seeking a Full Time RN Case Manager (RNCM) to plan, organize, and direct hospice care for patients in their homes and communities. The registered nurse is the central point for the care plan and mentors Home Health Aides.

Must hold a Texas RN license with 2+ years of experience, have reliable transportation, and a valid Texas driver’s license. Kinnser EMR experience preferred.

Formación

• Current Texas State license as a Registered Nurse.
• 2+ years’ RN experience.
• Reliable transportation and valid Texas Driver’s License.
• Auto liability insurance.
• Kinnser EMR experience.

Responsabilidades

• Performs comprehensive assessment for patient admission.
• Develops and updates the plan of care with physicians and clinicians.
• Participates in discharge planning.
• Provides skilled nursing care per the plan of care.
• Coordinates with other agencies to ensure continuity of care.
• Maintains timely documentation and records, including visits and OASIS.
• Protects patient privacy and confidentiality.
