Job summary

Elara Caring is seeking a Registered Nurse Case Manager to join our team, delivering Right Care at the Right Time in patients’ homes. You will work within a collaborative hospice/home health setting, leading care coordination and education across the Interdisciplinary Team.

This salaried role requires nursing licensure, at least 1 year of clinical experience, and willingness to travel up to 50% while providing compassionate, high-quality care.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment preferred.
• 50% travel required.
• Ability to frequently lift, push, pull, and move up to 50 pounds.
• Valid driver’s license and reliable transportation.

Responsibilities

• Ensures all activities align with the board of directors, executive team, and Hospice leadership vision.
• Assesses hospice care patients and families to identify needs evidenced by clinical data and evaluations.
• Assesses patient needs across physical, psychological, social, and spiritual factors.
• Communicates significant findings to the Clinical Supervisor, physician, facility, and other personnel involved in patient care.
• Counsels and includes patient, facility, and family in the Interdisciplinary Plan of Care.
• Teaches hospice philosophy and pain/symptom management to staff in alternate care settings.
• Provides support at time of death and performs bereavement assessment.
