Job summary

Elara Caring is hiring a Registered Nurse Case Manager to support home-based hospice care, coordinating interdisciplinary plans and ensuring high-quality patient outcomes. You will operate in patients' homes, collaborate with clinical teams, and participate in ongoing education and bereavement support.

The role requires a valid RN license, 1 year of clinical experience, and a willingness to travel up to 50% to serve patients across assigned areas. Reports to Clinical Team Manager.

Qualifications

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• 1 year of clinical care experience.
• Experience in hospice or home health is preferred.

Responsibilities

• Assess hospice care patients and families to identify needs as evidenced by documentation and on-site evaluations.
• Communicate significant findings and changes in health condition to the Clinical Supervisor and other personnel.
• Counsel and educate patient, facility, and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain management to staff in alternate care settings.
• Provide bereavement support at time of death.
