Job summary

Elara Caring seeks a Registered Nurse Case Manager to support hospice patients in home care. This salaried role requires strong clinical skills, compassionate communication, and coordinating care across disciplines to ensure the right care at the right time and place.

You will visit patients in their homes, assess needs, and collaborate with the Interdisciplinary Team. Travel up to 50% with a valid RN license and reliable transportation are essential.

Qualifications

• Graduate of an accredited School of Nursing (Certificate, Diploma, Associate, or Baccalaureate).
• Current State RN license.
• 1 year of clinical care experience.

Responsibilities

• Coordinates care with Interdisciplinary Team.
• Assesses hospice patients and families to identify needs.
• Communicates findings and changes in health condition to the team.
• Counsels and includes patient/family in care plan.
• Provides education on hospice philosophy and symptom management.
• Provides support at time of death and bereavement assessment.
