Job summary

Trustbridge Hospice, part of Empath Health, is seeking a compassionate Hospice Registered Nurse (RN) Case Manager to join our Palm Beach County team. Monday–Friday 8:30am–5:00pm with weekend requirement.

Trustbridge Hospice has served Palm Beach and Broward counties with Full Life Care since 1978, focusing on comfort, dignity, and family-centered support. We are a mission-driven, not-for-profit network under Empath Health.

Qualifications

Registered Nurse licensed in Florida.
Current Basic Life Support certification.
One year in hospice, public health, home health, oncology, medical/surgical, or geriatric nursing preferred.
Knowledge of hospice nursing: case management, crisis intervention, communication, teaching, thanatology.

Responsibilities

Serve as a key advocate for patients and families, ensuring needs are recognized and addressed.
Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team.
Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group).
Use clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life.
Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust.
