Job summary

Trustbridge Hospice in Palm Beach County is seeking an experienced RN Hospice Case Manager to coordinate patient and family care under the direction of the primary physician and the IDG. This PRN role offers weekend availability and a schedule from 8:30am to 5pm, with a focus on comfort, dignity, and Full Life Care for all.

You will identify patient and family needs, develop and follow individualized care plans, and use teaching and crisis intervention to maximize quality of life.

Qualifications

• Active Florida RN license.
• BSN is preferred.
• Hospice certification preferred.
• BLS certification required.
• 1 year in hospice, home health, or related nursing.

Responsibilities

• Coordinate patient/family care under the physician and IDG.
• Identify patient/family needs and develop care plans.
• Utilize monitoring and teaching to maximize comfort.
• Maintain confidentiality and provide compassionate service.
• Ensure timely addressing of concerns.
