Job summary

Phoenix Home Care & Hospice in Jefferson City, MO is seeking a dedicated RN Case Manager to provide compassionate hospice nursing care within an interdisciplinary team. You will develop and implement a patient/family centered plan of care, with goals of care discussions to guide healthcare decisions and quality of life.

Requirements include Missouri or Compact RN license and hospice or long-term nursing experience, plus a valid driver’s license.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and guidance with healthcare decisions.
• Consult with the attending physician and Hospice Medical Director regarding changes in patient condition and plan of care.
• Coordinate hospice care by working with team members and patients/family to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Perform evaluations for hospice care and informational visits.
