Job summary

Elara Caring in Dexter, MO is seeking a Registered Nurse Case Manager to coordinate hospice care for patients and families. In this full-time role, you will assess needs, monitor conditions, educate caregivers, and collaborate with a multidisciplinary team to deliver compassionate, comprehensive care.

You will travel between patient homes, facilities and hospital settings, ensuring timely communication with physicians and the care team, and supporting bereavement services for families.

Qualifications

• Active RN license in Missouri.
• At least 1 year of nursing experience in a clinical setting.
• Ability to lift up to 50 pounds and assist with patient mobility.
• Must have reliable transportation to visit patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families for physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and communicate changes to the care team promptly.
• Educate and support patients, families, caregivers, and facility staff on the Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom relief.
• Provide bereavement support to families.
