Job summary

The Hospice RN Case Manager at Dover Health will connect with patients in their homes to promote physical, emotional, and personal well-being and support independence where possible.

Responsibilities include performing nursing assessments, developing care plans with an interdisciplinary team, educating patients and families, supervising LPNs and CNAs, and traveling within the service area to deliver high-quality hospice care.

Qualifications

• Diploma, Associate or Bachelor’s degree in nursing
• Current RN license, specific to the state you are assigned to work.
• Hospice or home care experience strongly preferred.
• Willing to obtain hospice and palliative care certification within 1 year.
• Current CPR certification.
• Current and valid driver’s license, reliable transportation, and current liability auto insurance.
• Ability to work well with others and take direction from management, as well as take initiative.
• Self-motivated to accomplish identified goals with a strong sense of accountability for results.
• Proficiency in Internet browsers, Microsoft Office programs including Outlook, Excel, Word and Publisher, EHR systems.

Responsibilities

• Provide and direct nursing care as prescribed by the physician within laws and agency policies.
• Perform initial/comprehensive nursing evaluation and develop a plan of care with the team.
• Regularly assess patient needs and report changes; intervene as needed.
• Educate patients and families on illness management and home care.
• Promote patient safety, quality, and regulatory compliance.
• Participate in professional development and lifelong learning.
• Supervise LPNs and Hospice CNAs.
• Travel within the service area to deliver care.
