Job summary

Beth Israel Lahey Health at Home is seeking a Hospice RN Case Manager to plan and deliver care using the nursing process—assessment, planning, intervention, implementation, and evaluation. You’ll work with patients, families, and the interdisciplinary team while maintaining professional standards and clinical competency.

Responsibilities include initial assessments, developing care plans, patient education on the dying process and medications, and supervising Hospice Aides.

Qualifications

• Associate’s degree or BSN preferred.
• Active Massachusetts RN license.
• Active AHA BLS certification.
• At least 1 year of direct care nursing experience; EPIC experience preferred.

Responsibilities

• Conduct initial patient and family assessment to determine hospice care needs.
• Perform comprehensive physical assessments and review history of illness.
• Educate patient and family about the dying process and expectations.
• Develop a patient-specific plan of care with goals based on nursing diagnoses and pallia­tive actions.
• Instruct patient and family on medications, side effects, dosing, and routes; document services.
• Supervise Hospice Aides and provide feedback every two weeks at minimum.
• Participate in bi-weekly hospice case conferences.
