Job summary

Beth Israel Lahey Health at Home seeks a Hospice RN Case Manager to deliver compassionate, patient-focused hospice care. You will assess patients and families, plan individualized care, and coordinate with an interdisciplinary team to ensure comfort and quality of life during end-of-life care.

You will conduct initial assessments, educate patients and families about the dying process, and supervise aides while maintaining clinical competency and professional standards in a supportive home health

Qualifications

• Active Massachusetts RN license required.
• BLS certification required.
• 1 year of direct care nursing experience preferred in home health or public health.

Responsibilities

• Plan and deliver care to patients using the nursing process: assessment, planning, intervention, evaluation.
• Interacts with patients, family, and interdisciplinary team to maintain professional nursing standards.
• Complete initial patient and family assessment to determine hospice care needs.
• Perform comprehensive physical assessment and review history of illnesses.
