Beth Israel Lahey Health is seeking a Registered Nurse to plan, organize, and direct home care services in Massachusetts. The role emphasizes community health education and delivering care in patients’ homes according to home care regulations and payer guidelines.

Responsibilities include initial assessments, care plan implementation, medication administration, and supervising home health aides. OASIS completion and EPIC documentation are required.

Qualifications

• Active Massachusetts RN license.
• American Heart Association BLS certification.
• Associate’s Degree or BSN preferred from an accredited school of nursing.
• At least 1 year of direct care nursing experience; home care/public health preferred.

Responsabilités

• Plan, organize and direct home care services.
• Initiate and revise plan of care as patient needs change.
• Administer medications and treatments as prescribed.
• Instruct and supervise home health aide care every two weeks.
• Identify discharge planning needs in the care plan development.
• Maintain HIPAA confidentiality and agency guidelines.
• Complete OASIS documentation and EPIC timely entries.
