Job summary

PACS is seeking a Case Manager – Registered Nurse to coordinate and deliver patient-centered end-of-life care. The RN will serve as the primary clinical contact for patients and families, ensuring care is timely and aligned with goals and values.

Responsibilities include assessing needs, developing care plans with the IDG, providing direct nursing care and symptom management, and educating families about plans, medications, and progression.

Qualifications

• Active RN license in good standing.
• Minimum 2 years nursing experience with hospice/geriatrics.
• Experience providing direct care to patients with life-limiting illnesses.
• Strong assessment, critical-thinking and clinical decision-making skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently while collaborating in an IDG.
• Proficient with electronic medical records systems.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Coordinate all aspects of patient and caregiver care.
• Conduct comprehensive nursing assessments to identify needs.
• Develop, implement and update individualized care plans with IDG.
• Provide direct nursing care and symptom management.
• Educate patients and families about care plans and medications.
• Maintain ongoing communication with physicians and team members.
• Respond to changes in patient condition and adjust plans.
• Participate in on-call rotation for 24/7 patient support.
