Job summary

Cadre Family Companies is seeking an RN Case Manager to serve as the primary clinical advocate for patients, coordinating personalized hospice care that aligns with goals and unique needs.

You will collaborate with physicians, social workers, chaplains, aides, volunteers, and other care team members to ensure compassionate, high-quality care. If you’re passionate about meaningful hospice work, we’d love to meet you.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of practice.
• Minimum of one (1) year of nursing experience.
• Valid driver's license, reliable transportation, and the ability to travel throughout the assigned service area.

Responsibilities

• Complete comprehensive nursing assessments and develop care plans.
• Monitor patient progress and adjust care plans as needs evolve.
• Safely administer medications and educate patients and caregivers.
• Collaborate with interdisciplinary team to coordinate transitions of care.
• Provide emotional support and guidance during difficult conversations surrounding illness, grief, and end-of-life planning.
