Job summary

Hospice Services of Massachusetts in Plymouth is seeking a dedicated Hospice RN Case Manager to oversee patient and family needs, supervise hospice aides, and deliver professional nursing care using the nursing process.

You will develop individualized care plans, collaborate with interdisciplinary team members, and provide health care instructions while counseling patients and families to meet nursing needs. Weekly pay up to $50/hr, depending on experience.

Qualifications

• Graduate of an accredited school of nursing.
• Must possess an active and unencumbered RN license in Massachusetts.
• Prior RN hospice experience preferred.
• CPR certified.

Responsibilities

• RN supervises hospice aides.
• Complete comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care by utilizing all elements of the nursing process.
• RN develops and implements individualized care plans.
• Collaborate with interdisciplinary team members to ensure comprehensive care.
• Provide health care instructions to patients and families.
• Counsel patients and families on meeting nursing and related needs.
