Job summary

Hospice of Western & Central Massachusetts is seeking a full-time RN Case Manager to plan, organize and direct hospice care in patients' homes and communities. The role is on-the-road with mileage reimbursement and offers compensation up to $49 per hour with weekly pay.

The RN will perform comprehensive assessments, develop and manage care plans, supervise aides and collaborate with the Clinical Supervisor to coordinate care for an assigned caseload.

Qualifications

• Active RN license in Massachusetts is required.
• Bachelor’s degree in nursing is preferred, hospice experience is a plus.
• CPR certification is required or current.

Responsibilities

• Supervise hospice aides and coordinate patient care.
• Complete initial and ongoing patient/family assessments.
• Develop and revise plans of care with nursing diagnoses.
• Administer medications and treatments per physician plan.
• Counsel patients and families on nursing needs and care options.
• Coordinate care with clinical supervisor for assigned caseload.
