Job summary

Anthem Hospice West Valley seeks an experienced registered nurse to perform home health visits under physician orders and a patient‑centered plan of care. You will assess, educate, and supervise care while documenting outcomes and coordinating with the health team.

Qualified candidates hold an Arizona RN license (or a valid compact state license) and are prepared to manage start-of-care assessments, care planning, wound/medication reviews, and family education, all while upholding agency

Qualifications

• Current RN license to practice in Arizona or a valid compact state license
• Ability to assess and document patient needs and plan of care
• Strong communication with patients, families, and health team

Responsibilities

• Follow agency policies and standards of practice
• Provide skilled nursing services per physician orders and plan of care
• Document patient assessments, care provided, and responses
• Perform Start of Care assessments and ongoing care planning
• Educate patients and families about health conditions and treatments
• Coordinate with nursing and clinical staff to ensure continuity of care
• Maintain accurate records and ensure regulatory compliance
