Job summary

Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate Hospice RN Case Manager. You will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.

This on-the-road role covers Litchfield County and western Hartford County. Salaried with weekly pay up to $49/hour, depending on experience.

Qualifications

• Graduate of an accredited school of nursing.
• Active RN license in Connecticut.
• CPR certification.
• 1–2 years acute care experience in an institutional setting, and 1–2 years in home care.
• Bachelor's degree, with hospice care experience preferred.

Responsibilities

• Supervise hospice aides and provide professional nursing care utilizing all elements of the nursing process.
• Complete comprehensive assessments of patient and family to determine hospice needs and provide regular re-evaluations.
• Develop and implement care plans with goals based on nursing diagnosis and palliative actions.
• Counsel patients and families to meet nursing needs and provide health care instructions as appropriate.
• Act as Case Manager when assigned by Clinical Supervisor to coordinate patient care for the caseload.
