Job summary

Heart to Heart Hospice is hiring an RN Case Manager to enhance patient care for individuals with life-limiting illnesses. The role involves comprehensive assessments and care planning in collaboration with the patient and family, with on-call responsibilities and strict documentation practices.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and auto liability insurance.
• Two years of RN experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients with life-limiting illness and identify physical, psychosocial, environmental and ADL needs.
• Implement and revise the individualized plan of care.
• Coordinate with patient, caregivers, physicians and other healthcare professionals.
• Attend IDG meetings and participate in care planning and prioritization.
• Provide on-call coverage as assigned and ensure timely documentation.
• Comply with all laws, regulations and organizational policies.
