Job summary

Paycom - ATS is seeking an experienced Registered Nurse Case Manager to direct hospice care in Newburgh, IN. The role emphasizes community health education and coordinating patient and family needs in home settings.

Responsibilities include assessing patients, planning and revising care, communicating with physicians, and guiding families through care options while maintaining quality standards.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care in institutional settings.
• One to two years of home care experience preferred.
• Current licensure in State.

Responsibilities

• Assess patient and family to determine hospice needs; perform comprehensive physicals.
• Plan, implement and adjust the care plan with nursing standards and physician orders.
• Coordinate patient care, communicate with physician, and educate patient/family.
• Provide medications and treatments as prescribed; assist daily living activities.
• Participate in on-call duties and quality improvement activities.
