Job summary

At-Heart Home Care & Hospice seeks a dedicated RN Case Manager to deliver patient-centered hospice care in the Virginia Beach area. You will assess needs, develop care plans, and provide direct nursing care with an emphasis on comfort and dignity.

Join a supportive team with flexible scheduling and opportunities for advancement. You must hold an active RN license in VA, have reliable transportation, and be prepared for travel across the territory.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle insured as per state/organization requirements.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team and maintain documentation.
• Participate in on-call rotation as required by local branch.
