Job summary

Pathways Home Health and Hospice in Sunnyvale, CA seeks an experienced Registered Nurse Case Manager for hospice care, serving the San Francisco County area. This full-time role offers 8:30am–5pm hours with rotating weekends and holidays, and a $9,000 sign-on bonus.

You will admit patients, assess needs, develop a care plan, coordinate with a multidisciplinary team, and ensure quality and regulatory compliance while delivering compassionate, patient-centered care.

Qualifications

• Current California RN license is required.
• Minimum of one year of nursing experience required.
• Current CPR Certificate required.
• Current California driver's license and automobile insurance required.

Responsibilities

• Performs initial comprehensive assessments.
• Documents observations, clinical findings, problems, interventions, goals and discharge plans.
• Initiates and revises plan of care in consultation with Clinical Team Manager.
• Assesses need for services of PT, OT, ST, MSW and HHA.
• Provides and documents skilled care on all visits and teaches patient/family.
