Job summary

Pathways Home Health & Hospice is seeking a Registered Nurse (RN) Case Manager for Hospice in Sunnyvale. The role coordinates care for patients across the hospice program, admitting patients, assessing needs, and developing individualized care plans with a multidisciplinary team.

The position includes a 9:30am-6pm schedule with rotating weekends and holidays, serving patients in San Bruno to Redwood City territories.

Qualifications

• Current California RN license
• Minimum of one year of nursing experience
• Current CPR Certificate
• Current California driver’s license and automobile insurance
• Bilingual proficiency in Mandarin and English preferred

Responsibilities

• Performs an initial, comprehensive assessment.
• Documents observations, clinical findings, problems, goals and discharge plans.
• Initiates and regularly re-evaluates and revises plan of care in consultation with Clinical Team Manager.
• Coordinates care with PT, OT, ST, MSW and HHA and other team members.
• Attends and participates in the clinical team multidisciplinary patient conference.
