Pathwayshealth is seeking a Registered Nurse (RN) Case Manager for Hospice services in Sunnyvale, California. This full-time position involves assessing patients' needs, developing care plans, and coordinating with a multidisciplinary team. The posted compensation range of $56.95 - $78.30/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.
The Hospice Nurse admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.
