Job summary

Pathways Health in Sunnyvale, CA is seeking a Registered Nurse (RN) Case Manager for Hospice to join our team. The role requires coordinating care, documenting assessments, and developing individualized plans of care for hospice patients and families.

Full-time position with 8:30am-5pm shift and rotating weekends/holidays. Sign-on bonus available, with a posted pay range of $56.95-$78.30 per hour based on experience and qualifications.

Qualifications

• Current California RN license.
• Minimum of one year of nursing experience required.
• Current CPR Certificate.
• Current California driver's license and automobile insurance.

Responsibilities

• Performs an initial, comprehensive assessment.
• Documents observations, clinical findings, problems, skilled interventions, goals and discharge plans.
• Initiates and regularly re-evaluates and revises plan of care in consultation with Clinical Team Manager.
• Assesses the need for services of other team members (PT, OT, ST, MSW, HHA).
• Provides and documents skilled care on all visits and teaches patients and/or family.
