Pathways Home Health & Hospice in Sunnyvale, CA, is seeking an experienced RN Case Manager for hospice care. The role covers San Francisco County with a daytime schedule of 8:30am–5pm, rotating weekends/holidays.

You will admit patients, assess needs, coordinate with a multidisciplinary team, and document care plans. The position offers a $9,000 sign-on bonus and a competitive hourly compensation range ($56.95–$78.30/hour) with good faith expectations.

Formación

• Current California RN license.
• Minimum of one year of nursing experience.
• Current CPR certification.
• Current California driver's license and automobile insurance.

Responsabilidades

• Admit patients to service and assess physical, psychosocial, environmental and safety needs.
• Develop an individualized plan of care and document observations and interventions.
• Coordinate care with a multidisciplinary team and participate in quality improvement.
• Perform initial comprehensive assessments and regular plan-of-care revisions.
• Supervise Home Health Aide visits and document regulatory requirements.
• Communicate changes in patient condition to the team and document notes.
• Submit weekly visit schedules and assist with scheduling needs.
• Complete re-certifications, transfers, and discharges as needed.
• Attend clinical team conferences and participate in agency training and protocols.
