Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 whole-person care to patients and families in their homes.

You will coordinate individualized care plans with an integrated care team, educate families on care techniques, and support them through complex decisions with empathy and professionalism. Responsibilities include administering treatments, updating plans as needs evolve, and rotating on-call duties to ensure seamless care transitions.

Qualifications

• RN required with Associate, Bachelor, or Master of Nursing from an accredited program.

Responsibilities

• Provide 1:1, whole-person care to patients and families in their homes.
• Coordinate individualized care plans with an integrated care team.
• Educate patients and caregivers on care techniques, medications, and symptom management.
