Job summary

St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in their homes.

You will coordinate individualized care plans with an integrated care team and guide families through complex decisions with empathy and professionalism. Responsibilities include performing comprehensive assessments, developing care plans, administering medications, educating families on care techniques and symptom management, and rotating on-call

Qualifications

• RN with accredited nursing degree (AS/BS/MS).
• 1+ years of acute care experience; hospice/home health preferred.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
