Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes.

You will coordinate care plans with an integrated team, educate families, and support them through complex decisions with professionalism and empathy. The role requires an RN with an accredited nursing degree and 1+ year of acute care experience; hospice/home health is preferred.

Qualifications

• RN with accredited nursing degree (RN) required.
• 1+ year of acute care experience; hospice/home health preferred.
• Reliable transportation and a valid driver’s license.
• Ability to educate patients and families on care techniques.

Responsibilities

• Perform initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with patients and families.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, diets, and symptom management.
• Coordinate with the care team and rotate on-call duties.
• Support families through complex decisions with empathy.
