Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1 holistic care to patients and families in their homes.

You will collaborate with an interdisciplinary team to create individualized care plans, guide end-of-life discussions, and ensure compassionate, professional care. The role serves the Aitkin, MN area with a typical 8:00am–5:00pm schedule, full-time and on-call expectations, and a full benefits package including insurance, retirement, and professional development

Qualifications

• RN with accredited nursing degree (Associate/Bachelor/Master).
• 1+ years acute care experience; hospice/home health preferred.
• Ability to assess hospice needs and develop individualized care plans.
• Administer medications and educate patients/caregivers.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide 1:1 holistic home-based care for patients and families.
• Develop individualized care plans with the patient and family.
• Administer prescribed medications and treatments.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Rotate on-call duties and manage seamless care transitions.
