Qualifications

• One year of professional nursing experience in the past three years or a Bachelor's degree in Nursing.
• Experience in home health and/or hospice setting preferred.

Responsibilities

• Assess patient/family needs using the nursing process to plan and implement care.
• Coordinate patient/family services and prioritize needs with the transdisciplinary team.
• Coordinate care plan implementation and ensure care coordination and case management for each patient.
