Qualifications

• One year experience as a professional nurse in the past three years or a BSN from an accredited School of Nursing.

• CA RN license required; Public Health Nursing certification desirable; HPNA certification preferred.

Responsibilities

• Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions.
• Coordinate all patient/family services and prioritization of needs with the transdisciplinary team.
• Use a case management approach and refer to other services as needed.
• Coordinate the implementation of the plan of care and insure care coordination and case management for each patient.
