Job summary

Hopestone Hospice is seeking a dedicated RN Case Manager in Southeast Houston to lead coordinated, patient‑centered care at home. You will work with an interdisciplinary team to ensure high‑quality end‑of‑life services and advocate for patients and families.

The role requires an active RN license, CPR certification, and 2+ years Hospice experience. A valid driver’s license and HomeCare HomeBase familiarity are preferred, with a strong emphasis on compassionate communication.

Qualifications

• Active RN license.
• Current CPR certification.
• Valid driver’s license and auto insurance.
• Hospice RN experience preferred (2+ years strongly preferred).
• HomeCare HomeBase experience preferred.
• Strong clinical judgment and compassionate communication skills.

Responsibilities

• Complete initial and ongoing patient assessments.
• Develop, implement, and update individualized plans of care.
• Educate patients and families on disease progression and end‑of‑life support.
• Coordinate care with physicians, social workers, chaplains, and clinical teams.
• Supervise Hospice Aides and LVN/LPN support staff.
• Maintain accurate clinical documentation and communication.
• Participate in assigned on‑call rotation.
