Job summary

Affinity Hospice is looking for a registered nurse/case manager to plan, organize, direct, and provide hospice care. The nurse brings clinical nursing experience, with emphasis on community health education, to develop services for patients and families in their homes and communities.

Full-time position (40) hours per week. Minimum qualifications include RN license, diploma/associate/baccalaureate nursing, CPR, driving requirement, and 2 years hospice/home health experience.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Two years of RN experience in clinical care, home health or hospice preferred.
• Must be a licensed driver with insured vehicle and in good working order.
• Excellent observation and communication skills; nursing skills per competency checklist.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop and document patient plan of care.
• Provide care with OSHA infection control measures.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases to meet third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on-call duties as assigned.
• Fulfill other nursing duties to support department goals.
