Job summary

Resource 1 Home Staffing is seeking a Hospice Registered Nurse (RN) Case Manager in Platteville, WI. The role is 40 hours per week, providing comprehensive home-based hospice care with emphasis on community health education and patient-family support.

The RN Case Manager will coordinate patient care, perform assessments, develop and revise plans of care, and participate in on-call rotation as needed. A valid RN license and driver’s license are required.

Qualifications

• 1-2 years of recent acute care experience in an institutional setting.
• 1-2 years of recent home care experience (preferred).
• We’re excited to train RNs passionate about hospice care.

Responsibilities

• Assess patient and family to determine hospice needs and create an individualized plan of care.
• Provide professional nursing care following the nursing process and physician plan of care.
• Regularly re-evaluate patient and family needs and revise the care plan as status changes.
• Communicate with physicians and care team to coordinate patient care.
• Educate patients and families on self-care, medications, diet and home safety.
