Job summary

Resource 1 Home Staffing seeks a Hospice Registered Nurse (RN) Case Manager in Waukesha, WI for 40 hours per week to plan, direct, and coordinate hospice care in home and community settings. The role emphasizes patient and family education, assessment, care planning, and collaboration with an interdisciplinary team using Homecare Homebase and nVoq.

Eligibility includes an active RN license and clean driving record.

Qualifications

• Active RN license and driving license.
• 1–2 years of recent acute care experience in an institutional setting; home care experience preferred.
• Graduation from an accredited school of nursing.
• Comfort with hospice philosophy and end-of-life care.

Responsibilities

• Perform initial and ongoing patient/family assessments and develop plans of care.
• Provide professional nursing care using the nursing process.
• Coordinate patient care and participate in on-call duties.
• Educate patient and family on self-care and medication management.
• Maintain accurate and timely clinical notes.
• Ensure safe environment and continuity of care with the interdisciplinary team.
