Qualifications

• Graduate of an accredited nursing program and RN license in good standing.

• Ability to coordinate care and communicate effectively with patients, families, and the interdisciplinary team.

Responsibilities

• Assess patient and family needs and develop a care plan using nursing process.
• Provide professional nursing care and administer medications as prescribed.
• Coordinate patient care as case manager with the clinical team.
• Educate patients and families on self-care and disease management.
• Maintain accurate clinical notes and document changes in condition.
• Participate in on-call duties and ensure safety of patient environment.
