Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join our growing team in Martin, TN. This field-based role manages a patient caseload, coordinates care, and supports patients and families in the home setting.

Responsibilities include comprehensive nursing processes, data gathering on patient needs, and collaboration with an interdisciplinary team to ensure optimal care. Strong documentation and travel within Martin and surrounding areas are essential.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Manage a patient and family caseload across assessment, eligibility, admission, planning, implementation, and evaluation phases of the nursing process.
• Obtain data from patient physical, psychological, social, and spiritual factors to inform care planning.
• Coordinate with attending physician, interdisciplinary team, and other agencies for optimal care.
• Provide regular visits to reassess needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records, charts, and care plans.
• Supervise and document Home Health Aide delivery of care as required by guidelines.
