Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Martin, TN. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload across assessment, eligibility, admission processing, planning, implementation, and evaluation.
• Obtain data from patient factors (physical, psychological, social, spiritual) impacting needs and coordinate interventions.
• Initiate communication with attending physician and interdisciplinary team to coordinate optimal care.
• Provide regular visits to reassess needs and update care plans.
• Perform clinical pronouncement of death and support family.
• Maintain accurate patient records, charts, and care plans in timely manner.
• Supervise and document delivery of care by Home Health Aide per guidelines.
