Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join our growing team in Waynesboro, TN. This field-based role manages a patient caseload, coordinates care, and supports patients and families in the home setting.

The position requires RN licensure, CPR/First Aid, and a commitment to quality patient care. You’ll collaborate with an interdisciplinary team to develop and implement comprehensive care plans with regular visits and timely documentation.

Qualifications

• Must have graduated from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certifications.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess factors across physical, psychological, social, and spiritual domains that impact patient and family needs and coordinate interventions with the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to reassess patient needs and update the care plan.
• Perform clinical pronouncement of death and support the family.
