Addus HomeCare in Martin, TN is seeking an RN Case Manager to join our growing hospice team. This field-based role manages patient and family caseload, coordinates care, and supports home-based nursing services across Martin and surrounding counties.

The successful candidate will be a graduate from an accredited RN program, licensed to practice in the state, and comfortable working with an interdisciplinary team. Competitive benefits and a supportive work environment are offered.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data and assess information from patient factors that may impact needs and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and other agencies for coordinating optimal care.
• Provide regular visits to reassess the patient's needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans Timely.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.
