Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Waynesboro. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory includes Waynesboro, TN and surrounding counties. Schedule is Monday-Friday, daytime hours with on-call 4-8 times per month.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver's license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient physical, psychological, social, and spiritual factors that may impact needs and coordinate with the interdisciplinary team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide regular visits to re-assess patient needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by regulations.
