Qualifications

• Licensed registered nurse in the state of practice.
• Must possess CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Manage patient and family caseload across nursing process stages.
• Assess data from physical, psychological and social factors affecting care.
• Coordinate care with attending physician and interdisciplinary team.
• Provide regular visits to reassess needs and update the plan of care.
• Document and maintain accurate patient records and care plans.
• Supervise delivery of home health care by aides per guidelines.
