Job summary

Hospice of the Chesapeake in Waldorf, MD is seeking an RN Case Manager to provide in-home hospice care to patients and support their families. This full-time role offers 40 hours per week, Mon–Fri with one warm and one cold holiday per year, and opportunities to collaborate with social workers, chaplains, CNAs, and physicians.

Qualifications

• Maryland RN license or reciprocal license; BSN preferred.
• 2+ years in med/surg or home care; hospice/end-of-life preferred.
• Experience documenting in an Electronic Medical Record.

Responsibilities

• Provide nursing care and case management for life-limiting illnesses.
• Lead patient/family education and goals of care with the team.
• Collaborate with social workers, chaplains, CNAs and physicians.
• Ensure continuity and quality care from admission to bereavement.
