Job summary

Westminster-Canterbury on Chesapeake Bay, a not-for-profit life plan community, is seeking a Registered Nurse (RN) Case Manager for Hospice. This on-site, full-time role involves patient assessments, care planning, education for patients and families, and collaboration with the interdisciplinary hospice team.

The RN will supervise LPNs and Hospice Aides, ensure compliance, participate in performance improvement, and rotate on-call.

Qualifications

• At least one year of professional nursing experience.
• Unencumbered Virginia RN license (or compact state).
• Current CPR certification (Basic Life Support).
• Experience with Microsoft Office and Google Suite.
• Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurers.

Responsibilities

• Assess the patient's physical, functional, psychosocial, environmental, and daily living needs.
• Implement and revise individualized care plans and provide symptom relief.
• Educate patients and families on disease process, self-care, and end-of-life care.
• Initiate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes showing progress toward goals.
• Coordinate services and refer to other team members as needed.
• Inform physicians of changes in needs and outcomes of interventions.
• Determine scope and frequency of services based on acuity.
• Supervise LPNs and paraprofessionals following regulatory guidelines.
• Participate in training programs and hospice improvement initiatives.
• Rotate on call as needed; perform other duties as requested.
