Job summary

UnitedHealth Group, through its hospice network in Somersworth, NH, seeks an RN Case Manager to coordinate compassionate hospice care. You will assess patients, partner with the interdisciplinary care team, and develop care plans tailored to each patient's needs.

This role requires an active RN license in the state, CPR certification, and reliable transportation. You will participate in on-call duties and support families and caregivers while advancing high-quality care.

Qualifications

• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes and outcomes as appropriate.
• Educates patients, families, and caregivers in appropriate patient care techniques.
• Communicates regularly with the clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
