Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to cover North Lansing, St. Johns and Dewitt. This full-time RN role supports hospice patients and families in their home setting, coordinating care and leading an interdisciplinary team.

The ideal candidate will be a graduate of an accredited nursing program, hold an active RN license, and have at least 1 year of clinical care experience; hospice or home health experience is preferred. 50% travel is required.

Qualifications

• Graduate of an accredited nursing program.
• Current State RN license.
• At least 1 year of clinical care experience; hospice/home health preferred.

Responsibilities

• Assess hospice patients and families to identify needs, including physical, psychosocial, and environmental factors.
• Communicate findings and changes in health status to the clinical team and involved personnel.
• Counsel, instruct, and include patient and family in the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain management to staff, and provide bereavement support as needed.
