Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to cover North Lansing – St. Johns and Dewitt, with full-time schedule and up to 50% travel. You will assess patient needs, coordinate care with interdisciplinary teams, and support families in end-of-life care within the home setting.

The role emphasizes compassionate patient care, educating staff, and participating in bereavement support when needed. A nursing degree and RN license are required, with hospice experience preferred.

Qualifications

• Graduate of an accredited nursing program.
• Current state RN license.
• Minimum one year of clinical care experience.
• Hospice or home health experience preferred.
• 50% travel required.

Responsibilities

• Assess hospice care patients and families to identify physical, psychosocial, and environmental needs.
• Communicate significant findings and changes in health condition to the clinical supervisor, physician, facility, and team.
• Counsel, instruct, and include patient, facility, and family in the interdisciplinary plan of care.
• Teach hospice philosophy and pain and symptom management to staff in various care settings.
• Provide bereavement support at time of death.

As a Hospice Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

• Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assesses hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, interdisciplinary team reports, after‑hours reports, and on‑site evaluations.
• Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the clinical supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsels, instructs, and includes the patient, facility, and family in following the interdisciplinary plan of care.
• Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted‑living facilities, etc.).
• Provides appropriate support at time of death and performs bereavement assessment.

Requirements

• Graduate of an accredited certificate, diploma, associate, or baccalaureate school of nursing.
• Current state license as a registered nurse (RN).
• Minimum one year of experience in a clinical care setting.
• Experience in a hospice or home health environment is preferred.
• 50% travel required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license, insurance, and reliable transportation to perform job tasks.

You will report to the clinical team manager.
