Job summary

Bluegrass Care Navigators is seeking a Hospice RN Case Manager to provide compassionate, comprehensive nursing care to patients in home-based settings across Fayette and Jessamine County. The role requires an active RN license in Kentucky or a compact state, with experience in hospice philosophy and patient advocacy.

You will coordinate care with an interdisciplinary team and support families through challenging times.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or a Compact State.
• Minimum of one year of general nursing experience preferred; new graduate nurses will be considered.
• Demonstrates strong clinical nursing skills and the ability to maintain professionalism.
• Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
• Adheres to accepted professional, ethical, and legal standards of nursing practice.
• Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
• Maintains current CPR certification.
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with min requirements.

Responsibilities

• Deliver compassionate nursing care to Hospice patients in a home care setting or wherever the patient calls home.
• Develop and implement a holistic plan of care based on patient goals and preferences.
• Serve as patient advocate to coordinate care for patients, families and caregivers.
• Monitor and evaluate symptom management and adjust plans as needed.
