Job summary

Bluegrass Care Navigators is seeking a Hospice RN Case Manager to deliver compassionate, comprehensive home-based care to patients and families in Pike County and surrounding areas.

The role focuses on creating holistic care plans, active listening, and coordinating with an interdisciplinary team to ensure comfort, safety, and quality of life for patients in home settings and care centers as needed.

Qualifications

• Graduate of an accredited school of registered nursing.
• Currently licensed as a Registered Nurse in Kentucky or through a Compact State Board of Nursing.
• Minimum of one year of general nursing experience preferred; new graduate nurses will be considered.
• Maintains CPR certification and has access to a reliable vehicle.

Responsibilities

• Deliver compassionate and comprehensive home care to hospice patients.
• Create and implement holistic plans of care based on patient goals.
• Advocate for patients and coordinate care with families and caregivers.
• Monitor symptoms and adjust plans to maximize comfort and safety.
